~ No Call/No Show Fee (<24 hours in advance) $75 for insurance/Lien patients

~ Self pay and new patients will be charged the full price of the visit

Physical Therapy Evaluation &

Report Time

$250 (50-60 min)

$350 (60-90 min)*

Includes the following:

Evaluation of area of complaint, history, testing & screening for PT diagnosis with detailed report.
Discussion of Plan of Care
Education on home program
Treatment needed including dry needling, IASTM, cupping & manual therapy.
Telehealth services available for those injured or residing in Colorado
*All Motor Vehicle Accidents due to report time, referrals, provider collaboration, etc.

PT Treatment

$180 (45-60 min/

>2 diagnosis)

Includes the following:

Screening & Assessment needed for treatment
Dry needling, IASTM, cupping & manual therapy.
Review of neuromuscular or orthopedic home program and customized treatment progression
Therapeutic taping if needed
Telehealth Pricing same for 30 min session - Direction of care, guidance in neuromuscular home program, and education on diagnosis/tools for recovery.
